Step By Step Guide To POEA Online Appointment

Learn how to schedule an appointment online for important information.

Step 1:

Go to the official website.

Step 2:

Read the advice before signing up to learn how to use the online appointment system. After reading, click the “x” in the top right corner of the screen. You can also click the “Close” button to continue.

Make an Appointment

Step 3:

Pick the office location from the “Make New Appointment” box list. Click ‘Profile’ to see your current appointment. Next to it, find the ‘Make New Appointment’ form. Choose the brand location.

Step 4:

Now, choose the date and time for your appointment. Ensure you can be at the office on that date and time.

Step 5:

Pick your Mode of Deployment (Agency Hired, Direct Hired, Balik Manggagawa, Others). Select the Purpose of Request.

Step 6:

Enter your agency name and job site. Then, choose the OFW Type from the options.

Step 7:

Enter the start and end years for the records needed and click ‘Submit.’

Step 8:

You’ve successfully booked an appointment if you see a confirmation message.

Step 9:

Click ‘Print Appointment’ to get a printout of your appointment sheet.

Complete the online form to get the OFW Information Sheet.

For Filipinos working abroad (OFWs):

  1. Complete the form by filling in the necessary information.

2. Be careful when picking the Branch Office, Appointment Date, & Appointment Time.

Make it simple for people who may have little education. Follow these steps if family members or relatives need the OFW information sheet. First, write the name of the “Authorized Representative.” After that, go through steps 1 to 9 for the online appointment.

For Other Parties:

If someone asks for the OFW information sheet, they will need these documents:

1. A letter asking for the information was addressed to the Chief of the Central Records Division.

2. For individuals authorized by the OFW: Special Power of Attorney (SPA).

3. For government or private agencies: Proof that the representative is authorized to receive the record.

4. A Court Order to the POEA for parties with a court case.

5. An ID document with a picture (Passport, Driver’s License, PRC Card, NBI Clearance, Company ID, etc.).

6. Proof of any circumstances under Section 13 of the Privacy Data Act 2012.

7. Any other legal documents that may be required.

Follow these steps if you already have an account:

To sign in:

  1. Type your ARN, Mother’s Maiden name, and full name (First and Last Name).
  2. Check the box that says “I’m not a robot” to confirm you’re not a robot.
  3. Click the Login button to move forward.

If you forgot your ARN:

  1. Select ‘Forgot Appointment Reference Number (ARN)’.
  2. Fill in your Last Name, First Name, Middle Name, Date of Birth, and Mother’s Maiden name.
  3. Tick the “I’m not a robot” box to prove you’re human.
  4. Tick the Agree box.
  5. Press the ‘Login’ button to proceed.
