Application Requirements For Postal ID
Get the original and a photocopy of these documents:
I. Fill out two (2) PID application forms completely.
II. Show proof of identity with any of these:
- Birth Certificate from NSO or Local Civil Registry
- GSIS or SSS UMID Card
- Valid Driver’s License
- Valid Passport
- Marriage Contract for married women (if submitted documents don’t have the married name).
