What is the UMID Card?

The UMID Card is a special ID. UMID stands for Unified Multi-Purpose Identification; some call it UMID ID. It is like a regular ID, but it is used for four government groups: the Social Security System (SSS), the Government Service Insurance System (GSIS), and the Philippine Health Insurance Corporation (PhilHealth). The UMID Card started in 2011. It took the place of the old SSS ID from 1998. So, UMID is like the new SSS ID. People need to show their UMID cards when they deal with SSS.

The UMID Card has info about the person, like their name, address, and birthdate. It also has a photo and signature. Who can get a UMID Card?

  1. You can apply if you’re an SSS member with at least one contribution.
  2. You can get a new one if your UMID or SSS ID gets lost or damaged.
  3. You can update your card if your name or personal details need fixing.
  4. If you want your regular UMID card to be an ATM card, you can upgrade.
  5. If you have an old SSS ID, you can replace it with a new UMID card.

What do you need for a UMID ID?

To get a UMID ID, you have to give one valid ID. If you don’t have that, you must provide two other valid IDs. The IDs you give should be real, not copies.

Documentary Requirements (For Card Replacement Only)

If you need a new SSS card, you must give some documents and your ID. Here’s what you need:

If you lost your SSS ID or UMID card:

  • A paper that says you lost it (you can find a form to fill out here).
  • A paper that shows you paid some money.

If you need a new card because your information changed:

  • Show your old SSS ID or UMID card.
  • A paper that shows you paid some money.

If you didn’t get your UMID card in the mail:

  • A paper saying you didn’t get it.
  • A note or email from the Identity Management Department saying the delivery failed.
  • A paper that shows you paid some money.

If your UMID card is messed up, used as an ATM, or for other reasons:

  • A paper that shows you paid some money.

The paper proving you paid is called the “Miscellaneous Payment Form” or “Special Bank Receipt.” You can get this from the SSS-approved bank where you paid the ₱200 for the new card. You can also pay this at the SSS office when you go for your appointment.

How to Apply for Your UMID ID Card

You can get a UMID ID card from SSS in two ways:

  1. Visit a Branch:
    • Follow the Branch Number Coding System.
    • Go to your chosen SSS branch during your assigned day.
    • Check if your branch is listed for UMID applications.
  2. Online Appointment:

The Branch Number Coding System is in place at SSS branches in Metro Manila and cities like Baguio, Dagupan, Bacoor, Biñan, Cebu City, Lapu-Lapu City, Bacolod, Iloilo-Central, Cagayan de Oro City, and Davao City. Find your branch on the list of those accepting UMID applications.

Here’s the SSS coding schedule:

  • Look at the 10th digit of your SSS or Employer ID number.
  • Match it to the assigned day for walk-in appointments.
    • Example: If your 10th digit is 4, visit on Tuesdays.

Step 1: Get the UMID Form

Start by downloading the UMID form. Click here to see or save it.

If you’re on a computer using Chrome, the form will pop up in a new tab. Save it by clicking the download icon in the top right, or print it using the printer icon. You only need one copy.

If you got your SSS number online after December 10, 2020, you don’t have to fill out the UMID form. Set a meeting and head to the SSS branch for your fingerprint and photo. Remember to bring your printed SS number application and transaction slip on your appointment day.

Step 2: Complete the UMID Form

  1. Print the UMID form.
  2. Use a pen to write in capital letters.
  3. If info doesn’t apply, write N/A.
  4. Avoid mistakes; limit to 2 corrections.
  5. Sign near any correction.

  • Write your 12-digit CRN. If you don’t have one, just leave it blank.

SBR, Receipt Number, or Transaction Reference Number:

  • For card replacements, include your Special Bank Receipt (SBR), receipt number, or transaction number.
  • For initial enrollment, write “N/A” or leave it blank.

  1. Birthday: Put your birthday like this – Month/Day/Year. For instance, if you were born on October 15, 1990, write 10/15/1990.
  2. Birthplace: Write down the city, province/state, and country where you were born, just like it’s written on your birth certificate.
  3. Gender: Pick either “Male” or “Female.”

  1. Mother’s Maiden Name: This is your mom’s full name before she got married.

Address Information:

  • Home Address: Write where you live all the time. Write your province address if you live in the province but work in Metro Manila on weekdays. If you work abroad, do the same.
  • Postal Code: Also called ZIP code. If you don’t know it, check this list.
  • Country Code: Write PH, which is the code for the Philippines.

Personal Details:

  • Civil Status: Pick if you’re single, married, widowed, annulled, or legally separated.
  • Height (in centimeters): Use a calculator to change your height from feet and inches to centimeters.
  • Weight (in kilograms): Use a calculator to change pounds to kilograms.
  • Distinguishing Features: Write any special things on your face, like a mole. For example: “Mole under the right eye.” If you don’t have one, leave it blank or write “N/A.”

Other Card Applicant Data

Telephone Number:

  • Include the area code.
  • Leave blank if you don’t have a telephone number.

Mobile Number:

  • Write your 11-digit cellphone number.
  • If you don’t have a mobile number, use an immediate family member’s number (e.g., father, husband, daughter).

Email Address:

  • Write “N/A” if you don’t have an email.
  • An email address is required if applying from abroad.

Are You a Pensioner?

  • Check “Yes” if you are a pensioner.
  • Otherwise, check “No.”

Option for UMID Card as ATM Card or Regular UMID Card

  1. Regular UMID Card – Select this if you just want a regular UMID card.
  2. UMID Card as ATM Card – Choose this if you want a UMID card that can also be used as an ATM card.

Provide your bank details:

  • Bank Name – Write down your bank’s name (for example, Landbank).
  • Bank Branch – Specify which branch you prefer (for example, Landbank Intramuros) for card pickup.

Deceased Member Data

If your husband or wife who was an SSS member has passed away, only complete this part.

  1. SSS Number/CRN of the person who passed away.
  2. Name of the person who passed away.

Certification, Data Privacy Consent and Authorization

Read the privacy policy before signing the form. If you or the person applying can’t sign, put your fingerprints on it in front of an SSS staff member. They will also sign as a witness.

  • Your Name (Printed)

  • Date – Write the date you filled out and signed the form.
  • Fingerprints – Only use fingerprints if you can’t sign.
  • Witnesses for Fingerprints – SSS staff will fill this out.

Step 3: Book a UMID ID Appointment Online with the SSS

It’s time to schedule your appointment with the SSS to capture your biometric data for the UMID ID. To do this, follow these easy steps:

  1. Log in to your My.SSS account. If you haven’t created one yet, check out this article for guidance on how to sign up and activate your account.
  2. After logging in, go to the menu bar and click “MEMBER INFO.” Then, select “Appointment System” from the dropdown menu.

Go to the Member Appointments page. Fill in the info for your appointment:

  1. Purpose: Choose “Application for ID Card (UMID)” from the dropdown.
  2. Region: Pick the area where the SSS branch is (like Visayas).
  3. Branch: Select the SSS branch for your appointment.
  4. Appointment Date: Click the field, and a calendar will pop up. Grey dates are open. Click one to choose it.

Step 4: Go to the SSS Office for Your Appointment

On your appointment day, head to the SSS branch you picked to take your biometrics data. Bring one main valid ID, or if you don’t have that, bring two secondary valid IDs.

If you’re getting a replacement card, bring the needed documents. (like a Loss Affidavit or your old UMID ID) and the payment proof for the ₱200 replacement fee. You can pay this fee at the SSS branch for your appointment.

Dress appropriately with a collared shirt or blouse. Don’t wear accessories like necklaces, hats, piercings, or dangling earrings. Take off your eyeglasses and sunglasses before your appointment.

Always wear a facemask during your appointment. You can briefly take it off for your photo. Check the biometrics guidelines for more on health and safety rules set by the SSS.

After your photo and biometrics are taken, the SSS staff will tear off the acknowledgment stub at the bottom of your UMID form. Keep this stub for your records.

Step 5: Get Your UMID Card

1. Wait for a text from the SSS saying your UMID card is ready.

2. If you applied at an SSS branch, pick up your card there.

3. If you linked your UMID to an ATM, get it at your chosen bank within 30 days of getting the SSS text.

4. For regular UMID cards, claim them within 60 days of getting the SSS text.

5. Call the SSS hotline After 60 days to check if your card is still available.

6. They destroy unclaimed cards after five years.
