Application Requirements For Postal ID

Get the original and a photocopy of these documents:

I. Fill out two (2) PID application forms completely.
II. Show proof of identity with any of these:

  • Birth Certificate from NSO or Local Civil Registry
  • GSIS or SSS UMID Card
  • Valid Driver’s License
  • Valid Passport
  • Marriage Contract for married women (if submitted documents don’t have the married name).
